Sweet Brook Transition Care and Living Centers is a 184-bed skilled nursing center offering specialized Alzheimer and dementia care, and a transitional care and rehabilitation unit.  Accredited by the JCAHO.

We strive to restore and maintain the highest level of functional independence while preserving and enhancing each resident's personal dignity and quality of life. Our goal is to delay progression of chronic conditions, prevent acute illness and provide maximal comfort, together with the provision of personal services delivered in a caring and flexible manner that meets the ever- changing needs of our residents.
