Sweet Brook Transition Care and Living Centers is a 184-bed skilled nursing center and residential care facility offering specialized Alzheimer and dementia care, and a transitional care and rehabilitation unit.  Located in Williamstown, Massachusetts, Sweet Brook is accredited by the JCAHO (Joint Commission on Accreditation of Healthcare Organizations).

The Living Center  is well-equipped to offer physical and occupational therapy and other services to people who are expected to make a return to their homes. The rehabilitation services staff includes an occupational therapist, a physical therapist, a speech therapist, and assistants in each therapy category.

Those recovering from fractures, joint replacement, soft tissue injuries, multiple trauma, brain injury, stroke, and a wide range of other injuries and illnesses, can benefit from short-term stay or outpatient services.

At Sweet Brook, our goal is to delay progression of chronic conditions, prevent acute illness and provide maximal comfort, providing personal services delivered in a caring and flexible manner that meets the ever- changing needs of our residents, respectful of personal dignity and quality of life.
