The Social Service Department at Sweet Brook is a dynamic and important component of the Care Planning and Care Giving Team. There are three full-time licensed social workers who provide a range of medical/social work for the residents, families, friends and staff, as well as providing valuable outreach services to the community.

Social Service in extended care facilities is grounded in the theory and practice of professional social work. We have a legal obligation to adhere to government laws; we have a professional responsibility to provide good service; and, we have an ethical responsibility to seek change when laws unjustly affect our client population. Social workers foster a climate, which enables residents to retain their individuality, autonomy and dignity.

The social work team consists of a director and two other licensed professionals. Each team member carries a caseload and provides administrative, advocacy and clinical services to these individuals, their family members, and their friends.

The primary role of the social service staff is in the area of education. We create an atmosphere where residents are optimally cared for, respected and appreciated as individuals.
