The Transitional Care Center at Sweet Brook

The Transitional Care Center (TCC) at Sweet Brook provides a variety of sub-acute care services to meet the needs of a variety of patients.  Services include expanded physical and occupational therapy, speech therapy, rehabilitation-related physiatry consultation, on-site respiratory therapy, wound care, pain management services, and courtesy transportation for family members.

The TCC brings a depth of service that just isn't available anywhere else in our region.  We have assembled a very talented team to provide comprehensive post-hospital care to residents of the TCC.

Typical patients include those recovering from surgery or illness who no longer need acute care, but who need additional time to recuperate before returning home.  The average stay for a TCC resident can be 35 days or longer, offering the resident ample time to reach a state of improved health.

Rehabilitation services are a significant element in the TCC operation.  We work to be sure maximum benefit is gained by patients from the hospital-based and TCC-based caregivers.  It would not be unusual for a resident at the TCC to receive care from the same therapist who saw them at the hospital.

The TCC also offers on-site respiratory therapy services, a clinical service not offered by any other skilled nursing facility in the area.

A typical day for a resident of the TCC would include an assessment by nursing staff, aggressive therapeutic intervention, and activities.
